Vaccine Registration in Jefferson County
Registration is the first step toward receiving a COVID-19 vaccine, and applicants will be contacted as they become eligible. At this time, the following Jefferson County residents are eligible for vaccinations:
- People age 65 and up
- People who qualify for phase 1a – a group that includes vulnerable populations such as residents of nursing homes and long-term care facilities
- People who qualify for phase 1b
If any of these apply to you, you can expect to be contacted shortly after you register.
To register for the COVID-19 vaccine, you can go on the Jefferson County Emergency Management Agency website. Click on the green link for the vaccine information registration form that you will need to fill out.
When you fill out the online form, there will be sections where you can indicate your age group and whether you are considered “high risk.” This information will be used to determine when you will be eligible for the vaccine. If you are currently working, you can also indicate which line of work you are in, which may factor into your eligibility phase. If you have any special conditions or require transportation to your appointment, there is a “comments” section provided where you can indicate your needs.
All applications will be reviewed, and you will be contacted when your group is eligible. Once contacted, you will be given all the information you need to schedule your vaccination appointment.
If you need assistance filling out the form, you can call the Jefferson County Vaccine Call Center.
Vaccine Eligibility for Vulnerable Populations

High-risk medical conditions that qualify for phase 1c include:
- Cancer
- Chronic kidney disease
- COPD
- Heart conditions
- Being immunocompromised
- Having a solid organ transplant
- Having a BMI over 30
